Question (English)

Pernicious anaemia is which type of anaemia?

  1. Macrocytic hyperchromic ✓ Correct
  2. Microcytic hypochromic
  3. Macrocytic normochromic
  4. Normocytic normochromic
Explanation:
Correct Answer: Macrocytic Hyperchromic

Pernicious anaemia is a type of Macrocytic Hyperchromic anaemia (also called Megaloblastic anaemia). It is caused by Vitamin B₁₂ deficiency due to absent/deficient Intrinsic Factor (IF) from parietal cells.

Pathophysiology
  • No IF → Vitamin B₁₂ not absorbed in terminal ileum → deficiency
  • Vitamin B₁₂ is needed for DNA synthesis and maturation of RBC precursors
  • Without it, RBC precursors cannot divide properly → large, immature megaloblasts are produced
  • Large cells = Macrocytic; packed with Hb = Hyperchromic
Anaemia Type Quick Reference
Anaemia Type Cause
Pernicious Macrocytic Hyperchromic Vitamin B₁₂ / Folic acid deficiency
Iron deficiency Microcytic Hypochromic Iron deficiency
Chronic disease Normocytic Normochromic Chronic inflammatory conditions
